All forms are stored in a lockable filing cabinet and kept for seven (7) years from date of signature.                                                                                                                                                      

Use of Your Information

The Sound Sanctuary Norfolk obtains most of our information through the signing of consultation forms. Under no circumstances do we sell participant or personal information to others. Please note that we will not share health information that may have been provided in connection with a therapy treatment or class. With your consent, we may use certain information to provide you with information about The Sound Sanctuary Norfolk. This consent is optional and you can decide to withdraw it at any time.  

Information collected by The Sound Sanctuary Norfolk about its users is used internally by The Sound Sanctuary Norfolk, only for the following reasons:

 

• Collection of information on a Holistic Wellness Class Registration Form and Therapy Treatment consultation//waiver form including first name, last name, home address, telephone number, email address, emergency contact details, medical history, allergies, current medical condition including injuries or recent operations, prescribed medication and signature.

As a therapy treatment client, you will receive information and updates from The Sound Sanctuary Norfolk on a regular basis. This may include information on future classes/workshops, events, retreats, free classes and newsletters. We do our utmost to ensure these updates are relevant and we are mindful to not spam your inbox.
